1. Print
out our contingent-fee agreement, sign and fax it to A.I.M. at 708-442-6252.
2. Send us
copies of your original Workers' Comp policies for the past 3 years (not the
whole thing, just the pages that spell out the premium computation--usually this
is the first couple of pages of the policy.)
3. Send us
copies of the audit billing statements you've received for those same policies
as in 2.
4. Send us
copies of the audit workpapers for those audits if you have them in your files
(if you don't have them, it's ok--we can help you request copies as needed.)
5. Send us
copies of any experience modification factor worksheets you have in your files
(if you don't have any, don't worry--we can normally request copies from the
rating bureau if you give permission.)
6.Provide us with
a contact name and phone number (and email address, if possible) for an
appropriate person at your company so we can obtain information about your
company's operations as they may affect your classification for Workers' Comp
purposes.
